Çoklu Diş Eti Çekilmelerinin Modifiye Tünel Tekniği ve Modifiye Koronale Pozisyone Flep Tekniği ile Tedavisinin Karşılaştırmalı İncelenmesi

Periodontolojide iltihabi periodontal hastalıklardan sonra karşılaşılan en önemli rahatsızlıklardan birisi mukogingival sorunlar olmuştur. Hastalarda, dentin hassasiyeti, kök çürükleri ve estetik kaygılar gibi problemlere yol açan dişeti çekilmeleri sonucu açığa çıkan kökleri kapatmak, periodontolojide önemli amaçlardan biri haline gelmiştir. Yakın tarihli bir sistematik derlemede, çoklu dişeti çekilmelerinin tedavisi için üç referans teknik belirlenmiştir. Bu teknikler koronale pozisyone flep için standart prosedür; Zucchelli & De Sanctis tarafından tarif edilen, dikey vertikal serbestleştirici insizyonlar olmayan değiştirilmiş versiyonu; ve tünel flep hazırlığına dayalı tekniklerdir. Vertikal rahatlatıcı insizyonlar olmadan flep kaldırmanın kan desteğine ve yara iyileşmesine katkıda bulunduğu varsayılmaktadır. Ayrıca, doku yüzeyinde herhangi bir görünür insizyondan kaçınmanın, postoperatif skar dokusu oluşumu riskini en aza indirdiği için daha iyi bir estetik sonuç sağladığı açıktır. Bu olgu sunumunda farklı iki hastada aynı tip çoklu dişeti çekilmelerinin modifiye tünel tekniği ve Zucchelli & De Sanctis tarafından tarif edilen modifiye koronale pozisyone flep tekniği ile tedavisinin karşılaştırmalı olarak değerlendirilmesi gösterilmiştir.

A Comparative Evaluation of the Treatment of Multiple Gingival Recessions Using the Modified Tunneling Technique and the Modified Coronally Positioned Flap Technique

In periodontology, one of the most important problems encountered after inflammatory periodontal diseases has been mucogingival problems. Closing the roots exposed as a result of gingival recession, which causes problems such as dentin hypersensitivity, root caries and aesthetic concerns in patients, has become one of the important goals in periodontology. In a recent systematic review, three reference techniques were identified for the treatment of multiple gum recessions. The first technique is the standard procedure of coronally positioned flap. The second technique is a modified version without vertical relaxing incisions as described by Zucchelli & De Sanctis and the third one is techniques based on tunnel flap preparation. The assumption is that lifting the flap without vertical relaxing incisions contributes to better blood supply and wound healing. Additionally, avoiding any visible incisions on the tissue surface minimizes the risk of postoperative scar tissue formation, leading to a better aesthetic outcome. In this case report, a comparative evaluation of the treatment of the same type of multiple gingival recessions of two different patients with the modified tunnel technique and the modified coronally positioned flap technique described by Zucchelli & De Sanctis is shown.
