Local anesthesia is a method that is frequently used in dental clinical practice but can also cause difficulties in behavioral management by triggering anxiety and fear in pediatric patients. Anxiety that occurs during dental procedures in children can make treatment difficult. Fear of seeing or experiencing needle penetration, swelling in soft tissues, and long-term numbness, as well as allergic reactions to anesthetic drugs, are the most common factors that cause patients and dentists to worry about the use of local anesthesia. According to the American Dental Association, fear of pain may prevent patients from visiting dentists. Considering that severe fear and anxiety can increase the perception of pain, approaches with the potential to reduce this feeling of pain and discomfort are of great importance. Therefore, painless anesthesia is critical to ensure trust in dentists during the procedure. With the discovery of new alternative and atraumatic anesthesia methods, the quality of dental treatment in pediatric dentistry is increasing. The latest developments in anesthetic agents, devices, and techniques used in local anesthesia have resulted in increased pain control, decreased injection pain, and fewer side effects. Many methods have been used to relieve pain during injection, such as topical anesthetics, heating the anesthetic solution, or increasing the injection time. Recently, systems such as intranasal spray, lidocaine tapes, jet injectors (needle-free injection), vibration devices, computer-controlled local anesthesia systems, intraosseous anesthesia systems, lasers, electronic dental anesthesia, and virtual reality distraction have been proposed as alternatives to traditional anesthesia techniques to reduce pain and anxiety. Research on the reliability and effectiveness of these techniques is ongoing. The integration of these innovations into dental practice is still limited owing to their high cost and low accessibility. Since painless treatment is an integral element of a quality dental experience, dentists should be informed of new developments in local anesthesia. In addition to effective injection techniques, the use of age-appropriate terminology in children and adolescents and techniques such as distraction ensure the formation of trust between the dentist and the patient and support this relationship.
