Objective: This study evaluated the effects of nasal septal deviation (NSD) direction and severity on olfactory bulb (OB) and olfactory tract (OT) morphometry.

Methods: This prospective autopsy study included 86 cases aged 18-80 years. According to computed tomography (CT) findings, cases were classified into right NSD (n=46) and left NSD (n=40) groups. During autopsy, OB and OT length, width, weight, and volume were measured. Normalized volume ratios were calculated. CT images were analyzed with 3D Slicer to determine the NSD angle and tortuosity ratio. Cases were evaluated in four groups according to NSD severity.

Results: Left OB length was shorter in the left NSD group (p=0.049). OB width was lower on the deviated side in both groups (right NSD: p=0.008; left NSD: p<0.001). In the left NSD group, left OT length and width were reduced (p=0.003; p<0.001). Total OB+OT volume and normalized volume ratio were lower on the deviated side (p<0.05). OB and OT widths and tortuosity ratio differed significantly according to NSD severity (p<0.05). In the right NSD group, NSD angle showed a moderate positive correlation with tortuosity ratio (r=0.494; p<0.001).

Conclusion: NSD direction and severity are associated with lateral morphometric alterations in the OB and OT. Reduced OB and OT volume and width on the deviated side suggest that asymmetric nasal airflow may contribute to structural changes in central olfactory structures.
