Özet
Access to hospital care for congestive heart failure (CHF) patients is often costly and delayed, worsening outcomes. Remote patient monitoring provides a solution by continuously tracking health indicators and supporting timely clinical decisions. For this approach to be effective, data must be presented in a clear, actionable format rather than raw streams that disrupt workflow. This study applies a User-Centered Design methodology to develop intuitive visualizations of daily blood pressure and weight data for CHF patients. Through three iterative cycles of requirement definition, visualization development, and evaluation, the design was refined to meet clinicians' needs. The final visualizations proved highly effective, enhancing efficiency, usability, and clinical decision-making.
